Job Summary

The Patient Services Coordinator is responsible for developing and maintaining strong referral relationships, representing Healing Hands Healthcare with professionalism, and ensuring a seamless experience for patients and referral partners. The Patient Services Coordinator plays a key role in supporting patient-centered care by serving as a liaison between referral partners, physicians, and the clinical team.

The ideal candidate is relationship-driven, highly organized, and experienced in healthcare business development. You will proactively grow existing accounts, identify new referral opportunities, and manage all aspects of order coordination while consistently promoting Healing Hands Healthcare as a trusted and valuable partner in patient care.

Essential Job Functions & Responsibilities

  • Achieve monthly contribution growth goals in alignment with company objectives.
  • Build and maintain collaborative internal and external relationships by aligning business development efforts with office operations.
  • Establish and strengthen relationships with physicians, hospitals, skilled nursing facilities, discharge planners, social workers, and other healthcare professionals.
  • Educate referral sources, facilities, organizations, and consumers on the services provided by Healing Hands Healthcare.
  • Maintain in-depth knowledge of the local healthcare community, facilities, and referral networks.
  • Regularly schedule and conduct visits with referral sources to identify patients appropriate for home health services and increase agency census.
  • Stay current on agency market trends, coverage criteria, and industry regulations to ensure compliance.
  • Represent Healing Hands Healthcare at health fairs, community events, and civic or professional committees as requested.
  • Proactively cultivate new business opportunities and territories in collaboration with leadership.
  • Manage all aspects of order coordination and tracking, including sending, following up, and ensuring timely completion of signed orders in compliance with Medicare guidelines.
  • Notify management promptly of any compliance or documentation concerns.
  • Maintain working knowledge of community resources and assist customers with referrals when services are not provided by Healing Hands Healthcare.
  • Monitor and report on the cost-effectiveness of marketing and outreach efforts.

Position Qualifications

  • Bachelor’s degree or equivalent professional experience required
  • Experience working in a healthcare environment; clinical background preferred
  • Proven sales or business development experience within a healthcare setting
  • Strong organizational and problem-solving skills
  • Excellent communication skills with a high level of professionalism and interpersonal etiquette

Job Type: Full-time

Pay: From $70,000.00 per year
