PCP, Gerentologist and/or Neurologist Virtual Partner

Job Overview
A Gerontologist/PCP/Neurology Partner with Embrace Prevention Care helps to bring virtual care services provided to their dementia patients and their caregivers through Medicare’s new Guiding an Improved Dementia Experience (GUIDE).

GUIDE is a new CMS Innovation Center 8-year pilot that started July ’24. The goal of GUIDE is to provide preventive, longitudinal care to Medicare beneficiaries with dementia as well as support services to their caregivers to reduce hospitalizations and the need for skilled nursing.

This part time role augments the practice of a busy, forward-thinking PCP and/or Gerontologist, who wants to provide the best, proactive preventive care for their patients. GUIDE PCP/Gerontologist partners will be asked to complete three specific asynchronous clinical tasks- Attest to their patient’s dementia diagnosis, review and approve the care plan Embrace creates, and answer GUIDE-specific questions from our Care Navigators if they come up. Embrace Care Navigators are Nurse Practitioners.

Embrace provides a turnkey system to meet all the GUIDE requirements, led by our local nurse practitioners who serve as the Care Navigators. Our local NPs complete most of the work, aided by the Neurologist Partner's oversight and guidance as required. We anticipate the monthly workload for 100 GUIDE patients will be approximately 2.5 hours.

Requirements

• Active Medical License (MD or DO) in either Delaware or Pennsylvania
• Current practicing PCP/Gerontologist/neurologist with a minimum of 5 years of experience with Medicare patients
• Currently caring for a panel of patients with dementia
• Desire to provide better care and support to dementia patients and caregivers
