PCP Mobile APRN or PA-C Wound Care

CCHCS is seeking an immediate dedicated and compassionate 1099 Wound/Primary Care PA-C/APRN to join our healthcare team. The ideal candidate will provide high-quality patient care, focusing on preventive health measures, Wound Care, and chronic disease management. This role requires a strong clinical background, excellent communication skills, and a commitment to patient-centered care. Set your own hours and provide WC in patients home setting providing wound care debridement, chronic care management & comprehensive wound care. Areas are Gardnerville, Carson City, Minden, Dayton, Reno, Fernley, possible Fallon. Prefer experience with Tebra EMR. You are not paid for drive time, however you are paid mileage at current rate. You will also see geriatric patients for Primary Care, and chronic care management.

Duties

• Conduct comprehensive health assessments and physical examinations of patients.
• Diagnose and manage acute and chronic health conditions.
• Develop individualized treatment plans based on patient needs and preferences.
• Educate patients about health management, preventive care, and wellness strategies through effective health coaching.
• Collaborate with interdisciplinary teams to coordinate patient care effectively.
• Provide Wound Care both comprehensive and debridement.

Qualifications

• Valid PA-C/APRN license in the state of NV
• Will collaborate and work under Medical Director- Excellent clinical judgment and critical thinking skills- Strong communication and interpersonal skills to effectively collaborate with patients, families, and healthcare professionals- Ability to work independently as well as part of a team
• Strong understanding of medical terminology and coding practices.
• Ability to work independently while also being a collaborative team player.
• Commitment to continuous professional development and staying current with best practices in primary care.

Pay: $90.00 per hour
