Pediatric Case Manager (Bilingual)

About the Role

Faith Medical Services is seeking a compassionate, organized, and proactive Pediatric Care Manager to join our growing team. This role is vital in ensuring that our pediatric clients receive consistent, high-quality care through effective coordination between families, caregivers, and the clinical team.

As a Pediatric Case Manager, you will be the heart of our scheduling and communication efforts—matching caregivers with families, resolving scheduling challenges, and ensuring smooth day-to-day operations for both clients and staff. This role also require marketing for new referrals. Needed support and resources are provided.

Qualifications

• Minimum: High School Diploma or equivalent
• Preferred experience with the Georgia Pediatric Program (GAPP)
• Previous experience in healthcare, home care, or pediatric coordination preferred
• Strong compassion, patience, and communication skills
• Excellent organizational, problem-solving, and multitasking abilities
• Proficiency in Microsoft Word, Excel, PowerPoint.

Key Responsibilities

• Coordinate and manage daily schedules for pediatric clients and caregivers
• Handle all scheduling, call-outs, and rescheduling promptly and professionally
• Match caregivers to clients based on skills, availability, and personality compatibility
• Monitor missed clock-ins/outs and overall field staff performance
• Communicate regularly with case managers, physicians, and families
• Resolve client and staff concerns quickly and effectively, escalating as needed
• Conduct evaluations for field staff.
• Collaborate with HR on employee onboarding and orientation
• Track caregiver availability, overtime, and compliance
• Assist intake officer with new referrals and onboarding of pediatric clients
• Attend meetings, trainings, and client care conferences as needed
• Support billing, administrative, and quality assurance processes
• Provide weekly reports and updates to management
• Marketing skill is required.

Job Types: Full-time, Part-time

Pay: $19.00 - $21.00 per hour
