Pediatric Home Care Nurse RN 20 hrs per week

Join our compassionate team as a Pediatric Home Care Nurse RN, working 20 hours per week to provide specialized nursing care to children in their home environment. Responsibilities

  • Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations tailored for pediatric patients.
  • Administer medications, injections, IV infusions, and perform catheter care following aseptic techniques and HIPAA regulations.
  • Manage ventilators, oxygen therapy, tube feedings, and other specialized medical equipment in accordance with care plans.
  • Conduct wound suturing, dressing changes, ostomy care, and infection control procedures to ensure optimal patient outcomes.
  • Collaborate with families to develop and implement individualized care plans, including discharge planning and health coaching.
  • Document all patient interactions accurately using EMR systems such as Epic or Cerner while maintaining thorough medical records in compliance with HIPAA standards.
  • Coordinate with multidisciplinary teams for case management, triage assessments, and emergency response protocols in urgent or acute care situations.

Skills

  • Extensive pediatric nursing experience with familiarity in home & community care settings.
  • Proficiency in medical documentation systems like Epic, Cerner, or eClinicalWorks; strong EMR/EHR skills required.
  • Knowledge of complex medical procedures including IV insertion, medication administration, airway management, and specimen collection/processing.
  • Experience with specialized treatments such as botulinum toxin therapy or neurodevelopmental support for children with disabilities.
  • Critical care background including ICU or Level I/II trauma center experience; familiarity with trauma medicine and acute pain management preferred.
  • Ability to work independently while coordinating effectively with families and healthcare teams; excellent patient assessment skills essential.
  • Strong understanding of medical terminology, ICD coding (ICD-10/ICD-9), laboratory procedures, and infection control practices.
