Job Responsibilities

• Provide skilled nursing care in patients’ homes
• Perform assessments and monitor patient conditions
• Administer medications and treatments as ordered
• Tracheostomy, ventilator, and G-tube care (if applicable)
• Educate patients and families on care plans
• Complete, accurate, and timely documentation in compliance with Illinois regulations
• Communicate effectively with physicians and care team

Qualifications

• Active Illinois RN or LPN license (required)
• Current BLS/CPR certification
• Home health or private duty experience preferred (not required)
• Reliable transportation within service area
• Compassionate, dependable, and professional
