Job Summary
Serves as the primary organizational follow-up resource for individuals connected through encounters including the CRU, ensuring continuity of care, tracking progress post-referral, and maintaining sustained connection to services.
As a member of the Crisis Response Unit (CRU), bridges emergency services, mental health providers, social service agencies, law enforcement, and community partners to coordinate compassionate, effective responses for individuals experiencing mental health crises, substance use challenges, or urgent needs. Provides real-time support, facilitates referrals, and advocates for appropriate crisis interventions.
Key Responsibilities
Program Support & Implementation (40%)
- Serve as the primary organizational follow-up resource after crisis encounters and overdose incidents, ensuring continuity of care, tracking progress post-referral, and maintaining sustained connection to services
- Maintain accurate and timely updates in participant tracker documenting intake, updates, and issues
- Document client encounters in the Alliance system concurrently and daily to ensure continuity of care
- Maintain regular contact with clients via phone and in-person meetings to support engagement and recovery progress
- Support recovery programs including SMART Recovery and Resolve trainings
Collaboration, Coordination & Community Engagement (30%)
- Work directly with CRU officers during scheduled field times (up to 2 days per week) as identified by the Sergeant and Alliance Supervisor
- Coordinate client care with assigned Co-Responder
- Build and maintain law enforcement partnerships to support CRU operations, bridging communication between local and county agencies
- Represent the Alliance with professionalism and integrity in all interactions with partners, clients, community stakeholders, and public-facing activities
- Represent lived experience at Alliance, Coalition, and community events including health fairs and school functions
- Participate in staff meetings, case reviews, and community collaborations
- Submit timely updates, data, and outcome reports
Client Engagement & Support (30%)
- Connect individuals and families to appropriate services and resources
- Work in designated clinics as required. See Honor Health requirements below.
- Maintain current knowledge of community resources and referral processes
- Build rapport through lived experience and recovery-oriented approach
- Provide on-call support through designated Alliance system
- Demonstrate professionalism and appropriate boundaries in all client interactions
- Promote confidence, independence, and community integration through one-on-one support at all stages of the recovery process
- Provide moral, emotional, informational, and instrumental support, including assistance navigating systems such as DHS and Social Security
- Assist clients in building a recovery support network through support groups
Note: This role requires flexibility and responsiveness to client needs, with responsibilities that may shift based on program demands and client crises. All duties contribute to the overall mission of diverting individuals from the criminal justice system, supporting their successful reintegration into the community, and fostering positive recovery pathways toward health, stability, and hope.

Job Qualifications
- Education: Bachelor’s degree preferred
- Experience: 5-10 years of related experience
- Peer Recovery Coach Certification (CPRM & CPRC)
- Minimum of four (4) years of continuous recovery from substance use disorder
- Bilingual (English/Spanish) strongly preferred
- Experience in case management or care coordination preferred
Working Conditions
- Typical office environment with occasional travel.
- Must have a valid Michigan Driver's License, current automobile insurance and use of vehicle during working hours.
- May require travel to client homes, clinics, or community settings.
- Role may involve navigating emotionally charged or high-risk situations.
- May require flexibility in working hours to accommodate multiple schedules.
- Ability to lift and transport materials/supplies up to 40 lbs. to health fairs, presentations and/or other public forums.
- Ability to work in a mid-level noise collaborative environment.
- Ability to maintain normal office hours at the Alliance site as well as hours based on program need including some evenings and weekends.
Skills
- Strong Understanding of Substance Use Disorders, Recovery and Mental Health
- Planning, implementation and coordination of substance abuse prevention services
- Community organizing experience
- Case management experience, including documentation, referral tracking, and client follow-up
- Excellent written and verbal communication skills
- Self-motivated and organized
- Ability to manage multiple projects simultaneously
- Computer knowledge, especially Microsoft applications; must have experience with Excel and Google Sheets
- Cultural Competence and experience working with a diverse group of individuals
- Ability to attend meetings and represent the organization/coalition
- Interest and innovation in the “big picture” of public health and behavioral health issues
- Ability to present self in a courteous, tactful, and professional manner
- Considerate, punctual, and attentive to details
