Our patients are seen across diverse settings — private residences, skilled nursing facilities (SNFs), board-and-care homes, and assisted living communities.

Position Summary
We are seeking a Nurse Practitioner (NP) with a passion for wound management and patient-centered care. The ideal candidate is reliable, compassionate, and able to work independently while maintaining strong communication and coordination with our interdisciplinary team.

Area
Service coverage includes San Francisco, CA and San Jose, CA, serving patients in home and facility-based settings.

Key Responsibilities

• Provide wound assessment, treatment, and follow-up in home and facility settings
• Communicate and coordinate care plans with physicians, facility staff, and caregivers
• Maintain accurate and timely documentation in the EMR
• Collaborate with case managers and other healthcare professionals to ensure continuity of care
• Drive between patient locations within the assigned region

Qualifications

• Current and active California Nurse Practitioner license (FNP, AGNP, or equivalent)
• Reliable transportation and willingness to travel between patient locations
• Strong clinical assessment and bedside skills
• Excellent verbal and written communication abilities
• Comfortable using electronic medical record (EMR) systems
• New graduates are welcome – must be eager to learn, coachable, and committed to professional growth

Preferred Qualifications

• Experience in wound care, home health, hospice, or post-acute settings (preferred but not required)
• Wound care certification (WCC, CWCN, CWS, or similar) a plus
