Per Diem RN/LPN

Job Summary
The Adult Foster Care (AFC) Registered Nurse or LPN is responsible for providing ongoing health oversight and support to individuals receiving services under the MassHealth Adult Foster Care program. The AFC Nurse conducts required monthlyhome visits to evaluate each member’s health status, ensure the appropriateness of care, and support the member and caregiver in maintaining a safe and healthy home environment. The nurse works closely with the AFC Case Manager and interdisciplinary team to develop and monitor person-centered care plans in accordance with MassHealth AFC program standards.

Essential Duties and Responsibilities:

  • Conduct monthly home visits to assess each member’s physical, mental, and emotional health status.
  • Perform and document initial nursing assessments, annual reassessments, and ongoing evaluations as required by MassHealth AFC regulations.
  • Develop, implement, and update individualized care plans in collaboration with the AFC Case Manager, member, and caregiver.
  • Provide education and training to AFC caregivers regarding health management, medication administration, nutrition, safety, and emergency preparedness.
  • Identify and report any changes in the member’s health condition and coordinate appropriate follow-up with the member’s primary care provider or specialists.
  • Maintain thorough, timely, and compliant documentation in accordance with MassHealth and agency standards.
  • Participate in multidisciplinary team meetings to review member progress and care plans.
  • Provide on-call or telephone support as needed to address urgent member or caregiver concerns.

Job Type: Part-time

Pay: $100.00 per day
