To ensure quality and safe delivery of Primary Home Care services. To assist in the provision and coordination of all personal attendant services in accordance with Agency philosophy and established standards.


Essential Functions:

  • Provides supervisory oversight to Personal Care Attendants under the Primary Home Care department.
    • Ensures client services are provided in accordance with task assignments and conforming to Agency policy.
    • Periodically monitors service being provided onsite.
    • Maintains contact with office support staff to ensure appropriate services provided.
    • Meets with clients and family on occasions to ensure client satisfaction.
    • Reports any concerns, problems or issues to appropriate Agency staff.
  • Performs Primary Home Care Supervisory Visits.
    • Orients Personal Care Attendants to new assignments and clients' needs.
    • Explains work tasks and schedules.
    • Determines attendant's competency to perform tasks.
    • Provides information about program rules and Agency policies to Attendant and client/ family.
    • Provides training and/ or demonstration of tasks to Attendant.
    • Resolves problems and/ or misunderstandings between client and Attendant.
    • Evaluates Attendant job performance.
  • Completes required work documentation, processes and procedures.
    • Completes supervisory visit reports.
    • Documents Attendant orientations.
    • Communicates with appropriate case workers.
    • Documents information about client services, issues and resolutions while maintaining completed compliant forms signed by client.
  • Coordinates Client Care
    • Notifies authorized family members and or physicians of significant changes in client physical, mental or environmental safety.
    • Coordinates client care with Agency and other relevant programs.
  • Uses equipment and supplies safely and efficiently.
    • Demonstrates accurate and efficient utilization of equipment and supplies.
    • Communicates information regarding safe and efficient use or malfunctioning equipment and/or inadequate supplies to appropriate individuals.
    • Provides information about resources for needed equipment and supplies to clients and families.
  • Participates in professional growth and development.
    • Participates in Agency sponsored education programs.
    • Participates in community activities and education opportunities.
    • Participates in Quality Assurance programs.
  • Fulfill additional performance responsibilities as assigned and necessary.


Physical Requirements: Visual/hearing ability sufficient to comprehend written/oral communication. Ability to deal effectively with stress.

Working Conditions: Works in office areas as well as throughout Independent Living Facilities and residential homes. Sits, stands, bends, lift, and moves intermittently during working hours. Interacts with residents, family members, staff, visitors, government agencies/personnel, etc., under all conditions/circumstances. Is subject to hostile and emotionally upset residents, family members, staff, visitors, etc. under all conditions/circumstances. Is involved in community/civic health matters/projects. May be exposed to infectious waste, diseases, conditions, etc., including exposure to the AIDS and hepatitis B viruses. Communicates with the medical staff, nursing staff, and other unit supervisors. Maintains a liaison with residents, their families, support staff, etc. to assure that the residents' needs are continually met.

Qualifications: High School Diploma or Equivalent. Completion of two (2) years at an accredited college or university preferred or may substitute each year of college with supervisory experience at a Health Care facility, Home Care Agency or Community Agency.
