Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility to ensure comfort and dignity.
- Monitor patients’ health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals.
- Support medication administration following care plans while adhering to HIPAA regulations to maintain confidentiality.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary.
- Maintain a clean and safe environment by performing light housekeeping tasks including cleaning patient areas and ensuring hygiene standards are met.
- Use specialized equipment such as Hoyer lifts or resident’s personal assistive devices to safely transfer or reposition clients.
- Document care activities meticulously, update care plans regularly, and participate in team meetings to ensure coordinated support.
Requirements
- Proven experience in home health, assisted living, nursing homes, or group homes with a focus on patient observation and direct support.
- Knowledge of essential caregiving skills including assisting with ADLs, medication administration, and patient monitoring.
- Certifications such as First Aid, CPR, and HIPAA compliance are required; additional training in dementia care, Alzheimer’s care, or hospice care is a plus.
- Ability to lift heavy objects safely and perform physically demanding tasks while maintaining compassion and patience.
- Familiarity with working with individuals with developmental disabilities, autism spectrum disorder, or memory impairments like dementia is highly desirable.
- Strong communication skills for collaborating with healthcare teams and supporting residents’ social needs.
- Valid driver’s license and reliable transportation preferred for home visits; flexibility to work various shifts including evenings or weekends.
