Overview

In this vital role, you will provide dedicated assistance to promote independence, safety, and well-being in various settings such as private homes, assisted living facilities, group homes, and nursing homes. Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patients’ health by observing vital signs, medication administration, and overall condition to ensure safety and comfort
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
- Support clients with mobility using equipment such as Hoyer lifts or transfer belts to ensure safe movement
- Maintain accurate care plans, document patient progress, and communicate updates to healthcare teams in compliance with HIPAA regulations
- Provide companionship and emotional support to reduce feelings of isolation and promote mental well-being
- Perform light cleaning duties to uphold a safe and hygienic environment in accordance with health standards
Experience
- Prior experience working in assisted living, home health care, hospice care, or long-term care environments is highly desirable
- Proven ability to work with individuals with developmental disabilities, autism, dementia (including Alzheimer’s disease), or other memory-related conditions
- Knowledge of patient observation techniques, vital signs monitoring, medication administration, and care plan implementation
- Experience using mobility aids such as Hoyer lifts or other assistive devices for patient transfer
- Familiarity with HIPAA guidelines to ensure confidentiality and privacy of patient information
- Certifications in CPR, First Aid, and basic life support are preferred but not required; willingness to obtain necessary certifications is appreciated
- Ability to lift heavy objects safely and perform physically demanding tasks while maintaining compassion and professionalism
