We are seeking a compassionate and dedicated PRN In-Home Aide to provide exceptional care and support to clients in their homes. Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs and reporting changes to healthcare professionals
- Support medication administration following prescribed protocols and maintaining accurate medical documentation
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques
- Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities accurately and securely in compliance with HIPAA regulations
- Assist with catheter care, wound management, and hospice support as needed
- Operate medical equipment such as Hoyer lifts and perform patient transfers safely using proper techniques
- Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and preferences
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for clients
- Support clients’ emotional well-being through companionship and engaging activities within their home or community settings
Requirements
- Prior experience in home health, assisted living, nursing homes, or long-term care environments preferred
- Knowledge of medical terminology and caregiving best practices for seniors and individuals with disabilities
- Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as first aid or hospice training are a plus
- Ability to work independently while following detailed care plans and medical instructions
- Strong patient observation skills to identify changes in condition or behavior promptly
- Experience working with individuals with developmental disabilities or dementia is highly desirable
- Familiarity with EMR/EHR systems for documentation purposes
- Excellent communication skills to collaborate effectively with clients, families, and healthcare teams
- Valid driver’s license and reliable transportation preferred for community-based visits
