Duties

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
• Monitor patient health by observing vital signs and reporting changes to healthcare professionals
• Support medication administration following prescribed protocols and maintaining accurate medical documentation
• Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques
• Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities accurately and securely in compliance with HIPAA regulations
• Assist with catheter care, wound management, and hospice support as needed
• Operate medical equipment such as Hoyer lifts and perform patient transfers safely using proper techniques
• Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and preferences
• Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for clients
• Support clients’ emotional well-being through companionship and engaging activities within their home or community settings

Requirements

• Prior experience in home health, assisted living, nursing homes, or long-term care environments preferred
• Knowledge of medical terminology and caregiving best practices for seniors and individuals with disabilities
• Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as first aid or hospice training are a plus
• Ability to work independently while following detailed care plans and medical instructions
• Strong patient observation skills to identify changes in condition or behavior promptly
• Experience working with individuals with developmental disabilities or dementia is highly desirable
• Familiarity with EMR/EHR systems for documentation purposes
• Excellent communication skills to collaborate effectively with clients, families, and healthcare teams
• Valid driver’s license and reliable transportation preferred for community-based visits
