Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and record vital signs such as blood pressure, pulse, and temperature to ensure patient well-being
• Support medication administration following prescribed care plans while adhering to HIPAA regulations for privacy and confidentiality
• Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary
• Perform light cleaning and household chores to maintain a safe and comfortable environment
• Observe and report any changes in the client’s condition or behavior promptly to healthcare professionals or family members
• Use specialized equipment such as Hoyer lifts or resident’s personal assistive devices to ensure safe transfers and mobility

Skills

• Proven experience in home health, assisted living, nursing homes, or group home settings
• Knowledge of dementia care, Alzheimer’s care, memory care, and working with individuals with developmental disabilities or autism spectrum disorder
• Certifications such as CPR, First Aid, and Hoyer lift training are highly desirable
• Strong patient observation skills with the ability to monitor vital signs and recognize health concerns early
• Excellent communication skills for effective interaction with clients, families, and healthcare teams
• Ability to perform heavy lifting safely and handle physical tasks involved in patient transfer and mobility support
• Familiarity with care plans, medication administration protocols, and HIPAA compliance standards
