Hospital Pharmacist – Job Description Primary Responsibilities
- Medication Dispensing
- Accurately prepare and dispense prescription medications.
- Verify dosages, interactions, and patient allergies.
- Ensure compliance with hospital policies and legal regulations.
- Clinical Pharmacy Services
- Review patient medication charts.
- Monitor drug therapy for effectiveness and adverse reactions.
- Recommend dosage adjustments and alternative therapies.
- Participate in ward rounds with physicians.
- Patient Counseling
- Educate patients on medication use, side effects, and storage.
- Provide discharge counseling on prescriptions.
- Drug Information & Consultation
- Provide drug information to healthcare professionals.
- Advise on medication compatibility, especially IV drugs.
- Support antimicrobial stewardship programs.
- Inventory & Supply Management
- Manage medication stock levels.
- Ensure proper storage (including controlled substances).
- Monitor expiration dates and reduce wastage.
- Regulatory Compliance
- Maintain accurate records.
- Ensure compliance with pharmacy laws and accreditation standards.
- Participate in audits and quality assurance activities.
- Compounding & Specialized Preparation
- Prepare sterile products (e.g., IV medications, chemotherapy).
- Supervise pharmacy technicians.
Qualifications & Skills
- Bachelor’s or Doctor of Pharmacy (D Pharma)( B Pharma) degree.
- Valid pharmacist license.
- Strong knowledge of pharmacology and therapeutics.
- Attention to detail and accuracy.
- Excellent communication and teamwork skills.
- Ability to work in a fast-paced environment.
