The Pharmacist at Crescent Community Health Center is responsible for ensuring safe prescribing practices and accurate transcribing, dispensing, monitoring, and documentation of pharmacy activities. This role is crucial in procuring medications and health-related items, ensuring proper storage, and maintaining the medication use system to support the health and well-being of the community.
Key Responsibilities:
Provide safe and effective prescribing practices to ensure patient safety and optimal therapeutic outcomes.
Accurately transcribe, dispense, and document pharmacy activities to maintain compliance with regulatory standards and organizational policies.
Monitor medication therapy to identify and resolve potential drug-related issues, ensuring patient safety and efficacy of treatment.
Procure medications and health-related items, ensuring availability and accessibility for patient care.
Ensure proper storage conditions for all medications and health-related items to maintain their integrity and effectiveness.
Maintain the medication use system, including inventory management and quality assurance processes.
Collaborate with healthcare professionals to optimize medication therapy management and improve patient outcomes.
Participate in continuous professional development and complete the 340B university full-day program within the first year of employment to enhance knowledge and skills in pharmacy practice.
Required Education:
BS or PharmD obtained from an accredited School of Pharmacy
Completion of in-person, 340B university full-day program within the first year of employment
Required Experience:
Prior retail or general pharmacy experience
Experience in safe prescribing practices and accurate transcribing
Experience in dispensing, monitoring, and documentation of pharmacy activities
