Overview
Responsible for verifying medication orders, preparing, compounding, dispensing medications, and counselling patients in accordance with accepted standards and established pharmacy policies and procedures.
Responsibilities
- Reviews, evaluates, processes, and medication orders for demographic data (e.g., gender, age, and weight), appropriateness of therapy, dose, frequency, route, dosage form, interactions, allergies, related laboratory values, and possible drug interactions.
- Advises/counsels patients and answers questions about medications (including indication, possible side effects, interactions among different drugs as applicable to the area of practice).
- Documents interventions and recommendations/clarifications in the electronic Health Information System as applicable.
- Maintains control and security of all medications including controlled, narcotic substances and supplies. Maintains dispensing and recordkeeping as per laws and regulations.
- Provides drug information including but not limited to drug selection, dosage, interactions, monitoring, and side effects, etc to all health care providers.
- Supervises, precepts, checks, and monitors medication-related actions/performance of lower/junior pharmacists, residents, trainees/students.
- Assists and guides health care providers with issues related to the medication management and use system.
- Participates/assists in conducting performance/quality improvement projects.
- Follows all Hospital's related Policies and Procedures.
- Participates in self and others' education, training and development, as applicable.
Qualifications
- Education: Master's or Pharm.D./Bachelor's Degree in Pharmacy is required.
- Experience: Two (2) years of related experience with Master's, or four (4) years with Pharm.D./Bachelor's Degree is required.
