POSITION OBJECTIVE/SUMMARY:

The Pharmacist is responsible for the delivery of in/out patient pharmaceutical related services: supervision of technical, and/or support duties in the ordering, compounding, dispensing, storing, distributing, and control of drugs and other pharmaceutical products and in the provision of expert information and consultation on drugs and drug usage to staff and clients of the facility. May supervise pharmacy departmental staff in the provision of pharmacy services; plan and organize pharmacy operations within the general framework of established facility procedures and professional standards, resolve issues related to appropriate use of drugs, and coordinate pharmacy services with other patient care activities. May perform routine administrative duties as part of the overall service goals.


MINIMUM QUALIFICATIONS REQUIRED FOR THIS POSITION:

  • A valid license to practice as a Registered Pharmacist (California Board of Pharmacy).
  • At least three years experience as a Registered Pharmacist in an in/out patient healthcare environment preferred.
  • Must be organized, detail oriented and the ability handle confidential information and

interface effectively with all levels of management.

  • Posses excellent written, verbal communications and interpersonal skills.
  • Must have the ability to problem solve, meet deadlines and follow through on work

assignments through completion, in a fast-fast paced work environment.


GENERAL STATEMENT OF FUNCTIONS:

  • Responsible for the day-to-day operations of the in/out patient pharmaceutical related

services. May supervise the activities of the departmental staff (Pharmacy Technicians) to

ensure all policies and procedures are adhered to and participates in the education and

training of new departmental staff which may include: (Pharmacist and Pharmacy Technicians).

  • Participate in the development and continual monitoring of client drug therapy treatment
plans and client outcomes.
  • Perform client assessments and monitors the effects of drug therapy using clinical

symptoms, evaluating laboratory data, and other relevant clinical diagnostic techniques.

  • Participate in daily rounds and medical conferences with physicians in specialty medical

areas to evaluate client drug therapy regimens as assigned by Director of Pharmacy

Services.
  • Interview clients to obtain information regarding drug use, drug allergies and sensitivities,
medication histories; assesses compliance and counsels patients on appropriate drug use.
  • Participate in ongoing medication usage evaluation programs by assisting in the

establishment of criteria for cost effectiveness/control utilization use, process indicators,

and monitoring parameters; conducts evaluations to determine compliance to the established

medication use evaluation criteria; summarizes results and submits recommendations to the

Director of Pharmacy Services and assists in establishing drug monographs.
  • Participate in the establishment of critical pathways to ensure optimal medication usage.
  • Provide comprehensive drug information services for healthcare providers and clients

including developing pharmaceutical newsletters, client education materials, and assessment

and evaluation of formulary requests to the Pharmacy and Therapeutics Committee.
  • Analyze, evaluate and prepare recommendations of medical literature in relation

to medication therapy.

  • Ensure client needs are met in a timely manner by filing all Treatment Authorization
Requests (TARS) on behalf of the patient as needed.
  • Advise and assist in implementing cost containment strategies for the usage of
pharmaceutical products.
  • Participate in performance improvement activities in ensuring optimal pharmaceutical care.
  • Practice confidentiality in all client, employee, and management related matters.
  • Assist in preserving Kedren’s mission, goals, and objectives; and assist management in

establishing its internal service delivery goals and objectives that are consistent with the

organization’s public service mission statement.

  • Participate in the fundraising activities of Kedren.
  • Assist in other duties and special projects as assigned.
