JOB DESCRIPTION
Position Summary:
The purpose of this position is to improve population based and patient specific medication related outcomes through collaboration with a multi-disciplinary medical team in the ambulatory setting. The pharmacist assumes a primary responsibility and accountability for the provision of safe and effective drug therapy and other medication management services intended to achieve outcomes related to the mitigation or prevention of disease, reduction of patients’ symptoms, prevention of adverse drug events and improvement of patients’ quality of life.
At the Burnsville clinic, the pharmacist will support population health initiatives through the improvement of Medicare Star measures in collaboration with a pharmacy technician. The areas of focus will be on improving medication use related to polyanticholinergic burden, statin use in targeted populations and improvement of key adherence measures. Once credentialed, the pharmacist will also provide direct patient care through the provision of comprehensive medication management.
