Job Summary and Qualifications

Position Summary: Under general supervision of a Pharmacy Manager, the Staff Pharmacist is responsible for directly overseeing and participating in all aspects of the hospital medication use system. Must take personal responsibility and accountability for patient medication-related outcomes. Should represent themselves, the pharmacy department, and the medical center in a positive and courteous manner, assisting all customers promptly. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. Pharmacy services are provided 24 hours a day/seven days a week. Schedule may vary according to the needs of the medical center/department and may include weekend, shift, holiday, on-call, and additional hours.

Job Responsibilities

  • Demonstrates the ability to adapt technique/treatment to age of population served, including newborn, pediatric, adolescent, adult and geriatric patients, while providing care/service with respect to cultural differences and spiritual needs.
  • Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles timely and accurately; maintains accurate, complete patient medication profiles.
  • Delivers clinical, patient medication problem-based care according to the standards defined within the American Society of Health -System Pharmacist (ASHP) and American College of Clinical Pharmacy (ACCP) Standards.
  • Acquires and maintains current knowledge in pharmacy practice.
  • Contributes to the professional development of peers, colleagues, and others.
  • Decisions and actions on behalf of patients are made as a patient advocate.
  • Collaborates with the patient, significant other, and healthcare providers in providing patient care.
  • Considers factors relating to safety, effectiveness, and cost of evaluating, planning and delivering patients care.
  • Supports and promotes the organization’s safety and risk management programs.
  • Complies with hospital and departmental policies and procedures, state/federal laws, regulations promulgated by the government or other third parties, TJC and ASHP standards.
  • Recommends drug discontinuation or dosage alteration when indicated (Within or below 1.5 standard deviations of the mean on rule override occurrences where there is no documentation).
  • Evaluates drug therapy orders for appropriateness of dosage, route, interval, schedule, and duration throughout patient's hospital course; makes recommendations for change when appropriate.
  • Detects and reports suspected adverse drug reactions accurately and in a timely manner (expectation of 2-5 ADRs).
  • Obtains and uses clinical lab data to evaluate appropriateness of drug product selection and/or dosage regimen (Within or below 1.5 standard deviations of the mean on rule override occurrences where there is no documentation).
  • Contributes to the quality and effective operations of the department.
  • Completes and documents inspection of liaison areas, if assigned.
  • Effectively communications oral and/or written therapeutic recommendations or other aspects of drug therapy to patients, i f assigned (expectation of 600 -1500).
  • Selects drug products or processes that are effective, cost beneficial, and promote patient compliance.
  • Participates actively and consistently in pharmacy programs designed to reduce medication- related costs (i.e. IV-to-PO switches, therapeutic interchanges, etc.) (expectation of 40-100 meets).
  • Identifies and assists with implementation of core practice changes within the assigned area (expectation of 2-4 scenarios).
  • Provides documentation of unique interventions or interactions with physicians that significantly impact the care of a patient; excluding situations identified by Meditech, policy and procedure, etc. (expectation of 3-7).
  • Performs all other duties as assigned.

Required Registration, Licensure or Certification

  • Current licensure as a Registered Pharmacist in the State of Kansas
  • Current Basic Life Support (BLS) verification course through American Heart Assoc. or American Red Cross - Healthcare Provider CPR (WFH Pharmacists exempt)

Preferred Registration, Licensure or Certification

  • Advanced Cardiac Life Support (ACLS) verification course through American Heart Association “ACLS Provider” or American Red Cross
  • Pediatric Advanced Life Support (PALS) verification through American Heart Assoc. or American Red Cross
  • Board Certified as Pharmacotherapy Specialist or other recognized specialty practice area Registered as a preceptor with the Kansas Board of Pharmacy

Required Education or Experience

  • Bachelor of Science in Pharmacy

Preferred Education or Experience

  • Doctor of Pharmacy (PharmD) degree
  • Pharmacy Residency Program (PGY1)
  • Pharmacy Specialty Residency Program (PGY2) Previous hospital experience

Preferred Knowledge and Skills

  • Demonstrated ability to communicate effectively
  • Knowledge of State and Federal laws, TJC, and professional practice standards
