Pharmacy Technician, Medication Reconciliation

Position Summary:

The Medication Reconciliation Pharmacy Technician (Med Rec Tech), under the supervision of a pharmacist, is responsible for obtaining an accurate medication list for pharmacy patients needing a medication reconciliation due to discharge from a facility, ensuring accurate and timely processing of prescriptions for these patients, and providing exceptional customer service. This role involves collaborating with healthcare providers, patients, and insurance companies to ensure excellent pharmacy services.

  • Shift hours: Ability to work 8am to 4pm or 9am to 5pm with rotating weekend availability

This role is Hybrid – Training will be in person with the possibility to be mostly remote depending on performance. In office days may be required for team meetings or trainings.

Essential Duties and Responsibilities:

The essential functions include, but are not limited to the following:

  • Obtain comprehensive medication histories for patients tagged as requiring medication reconciliation by contacting providers’ offices and/or caregivers via phone.
  • Accurately update patient profiles in pharmacy software to reflect current medications, document changes in dose/frequency/route and flag interactions for pharmacist review.
  • Communicate with patients and caregivers to coordinate timely delivery of medications.

Minimum Qualifications (Knowledge, Skills, and Abilities)

  • Required Qualifications: MA Registered Pharmacy Technician with National Certification (PTCB)
  • Previous experience with medication packaging automation or similar pharmacy technology preferred
  • Experience, or internship experience in a retail or fulfillment pharmacy
  • Must be compassionate, courteous and have excellent interpersonal skills
  • Detail oriented and the ability to handle multiple tasks effectively
  • Familiarity with Microsoft Office Suite, proficiency with CRM and a desire proprietary software application
  • Ability to adapt in a dynamic work environment, learn quickly, solve problems, and make decisions
  • Responsible for continuing education credits necessary to maintaining licensure
