Physiatrist, PMR Physician- Skilled Nursing

Job Summary
We are seeking a dedicated and energetic Physiatrist, specializing in Physical Medicine and Rehabilitation (PMR), responsible for providing rehab and pain consults in SNFs, including trigger points and joint injections. Your expertise will support patient recovery through innovative therapies, interdisciplinary collaboration, and compassionate care delivery. Responsibilities

• Provide physiatry consults in skilled nursing facilities
• Diagnose and manage functional, chronic, and pain-related conditions
• Develop and adjust individualized treatment plans
• Perform trigger point and joint injections
• Collaborate with therapy, nursing, and medical teams to coordinate care

Requirements

• MD or DO
• Indiana medical license or ability to obtain
• Board Certified or Board Eligible in PM&R
• DEA License
