Physiatry/Pain Management Provider (Physician or Nurse Practitioner)
Independent Visits | Post-Acute Care
Schedule: Full-Time or Part-Time | Flexible | No Nights or Weekends
Compensation: Competitive and based on credentials, experience, and schedule
MD/DO: $180,000 – $220,000 annually
NP: Competitive compensation (discussed during interview; based on experience and autonomy)
About the Role
Medley Health is seeking a compassionate, motivated Physician or Nurse Practitioner to join our growing post-acute care team. This role provides on-site physiatry and pain management services across skilled nursing and rehabilitation centers. You’ll work independently while collaborating closely with interdisciplinary teams to help patients regain function, improve mobility, and manage pain effectively.

Key Responsibilities
Perform comprehensive physiatry/rehabilitation evaluations and follow-up visits in SNF/Rehab settings
Diagnose and manage musculoskeletal, neurologic, and functional limitations common in post-acute populations
Develop and manage individualized treatment plans focused on:
Functional restoration and mobility goals
Pain management strategies
Coordination with PT/OT/ST plans of care
Monitor patient progress and adjust care plans as clinically appropriate
Document assessments and progress notes accurately and in a timely manner
Communicate recommendations to facility clinical teams and assist with discharge planning when needed
Qualifications — Physician (MD/DO)
MD or DO; Board Certified or Board Eligible in Physical Medicine & Rehabilitation (PM&R) preferred, or relevant experience in pain management, MSK, or rehab medicine
Active New York medical license
Experience in post-acute care, rehabilitation, pain management, or related settings preferred
Strong clinical judgment, documentation, and collaboration skills
Qualifications — Nurse Practitioner
Nurse Practitioner (Adult-Gerontology preferred; FNP considered with relevant experience)
Active New York NP license
Experience in post-acute/SNF care, rehabilitation medicine, pain management, geriatrics, orthopedics, or internal medicine preferred
Ability to work autonomously, manage a patient panel, and collaborate with interdisciplinary teams
Strong documentation and communication skills
