Physical / Occupational Therapist - Per Diem

Home Healthcare Agency seeks resourceful Physical / Occupational Therapists with demonstrable case management experience to serve as PER DIEM therapists. This position is ideal for qualified PT/OT seeking flexible work or incremental hours. We offer competitive pay $100 per session and limited travel – all service calls are within Philadelphia County. POSITION RESPONSIBILITIES:

The Physical / Occupational Therapist must be able to evaluate the patient’s condition and develop an individualized plan of care tailored to each patient based on the physician’s orders. Help patients manage pain and improve mobility, stability, balance, safety, and gait training to improve quality of life.

Key Leadership, Management, and Responsibilities for the PT/ OT include:

· Provide physical therapy services to patient according to a physician’s written orders during home visits to clients in within Philadelphia County

· Develop and initiate physical therapy Plan of care and instruct other personnel and/or family members and caregivers in home exercise program and progression.

  • Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
  • Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
  • Continually assess and revise the care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
  • Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.

· Accurately document observations, interventions, and evaluations pertaining to client care management and services provided and report patient’s response to treatment or changes in condition to the Director of Nursing and Physician.

PHYSICAL / OCCUPATIONAL THERAPIST QUALIFICATIONS MUST INCLUDE:

  • A current State of Pennsylvania Physical or Occupational Therapist (PT/OT) license.
  • A minimum of two years of appropriate work experience as a Physical / Occupational Therapist in a Facility or Home Health environment.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma
  • Evidence of compliance with all state mandated Physical Therapy Practice Act and other legal mandates

· Excellent customer service skills and ability to deal tactfully with patients, and staff.

  • Ability to work independently and manage time effectively and safely manage patients utilizing proper body mechanics
  • Strong interpersonal, organizational, and problem solving skills.

· Strong knowledge of principles, theory, methods and evidence based practice

  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.

· Compliant with continuing education, within state requirements as a condition of maintaining license.

· Comply with accepted professional, ethical and legal standards of practice.

· Must treat patients, families and staff with courtesy and respect.

· Adhere to HIPAA and maintain patient confidentiality.

ENVIRONMENT:

Contact with clients under a wide variety of circumstances. Care provided in client’s living environment with varying situations or in an institution such as a senior living facility or private residence. May be required to respond to emergency situations. Position includes tasks that have the potential for exposure to blood and other potentially infectious material (OPIM) such as body fluids/tissues.

TRAVEL REQUIREMENTS:

· Limited travel as needed for various staff and client care service needs.

· Up to 10% or as directed based on the needs of the business and attending job required training and conferences.
