Pleasant Acres Rehabilitation and Nursing Center is seeking a compassionate and skilled Physical Therapist (PT) to join our in-house rehab team! This is an exciting opportunity to work in a collaborative environment where patient care comes first and professional growth is encouraged.

Key Responsibilities:

  • Evaluate and develop individualized treatment plans for residents with temporary or chronic physical conditions.

  • Deliver skilled physical therapy services to promote healing, reduce pain, and improve mobility and function.

  • Monitor and document patient progress, adjusting treatment plans as needed.

  • Educate patients, families, and caregivers on home exercise programs, injury prevention, and rehabilitation techniques.

  • Collaborate with nursing, therapy, and medical teams to ensure coordinated care.

  • Complete documentation accurately and in compliance with company policies and regulatory guidelines.

Clinical Duties Include:

  • Performing evaluations and interpreting test results.

  • Implementing a variety of therapeutic treatment techniques.

  • Advising patients on proper movement, ergonomics, and lifestyle habits.

  • Developing personalized, goal-driven rehabilitation programs.

  • Ensuring adherence to all applicable rules, procedures, and state/federal regulations.

What We’re Looking For:

  • Licensed Physical Therapist in the state of Pennsylvania (or eligible)

  • Experience in Skilled Nursing Facility (SNF) or long-term care preferred, but new grads welcome!

  • Strong clinical skills and a passion for delivering high-quality, ethical care

  • Excellent communication and interpersonal skills

  • Organized, self-motivated, and a true team player
