Sulphur Springs Health and Rehabilitation is searching for a Full Time Physical Therapist to provide direct patient care to our DIVERSE geriatric community

Monday- Friday and Tuesday-Saturday Schedules Available

35-40 hours a week

Responsibilities:

• Evaluating and Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.

• Provides skilled physical therapy services/interventions in accordance to setting orders and communicating with the PTA

• May Implement and modify the patient’s physical therapy plan of treatment .

• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.

• Provide clinical advisement and support to the PTA and education to patients, family members, and caregivers.

• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.

• Attend required meetings as designated by the Director of Rehab.

Requirements:

• SNF/Long term care experience preferred

• Current knowledge of treatment practices

• Knowledge of Net Health Electronic Documentation System preferred
