Job Overview
We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. Your expertise will support patients across various settings, including inpatient hospital units, outpatient clinics, home health environments, and community care programs. Duties
- Conduct comprehensive patient evaluations to determine physical therapy needs, utilizing your knowledge of physiology and anatomy.
- Develop individualized care plans tailored to each patient’s condition, goals, and medical history.
- Implement therapeutic interventions such as exercise routines, manual therapy techniques—including trigger point therapy—and modalities to promote healing.
- Monitor patient progress regularly and adjust treatment strategies accordingly to ensure optimal outcomes.
- Collaborate with multidisciplinary teams—including physicians, nurses, and other healthcare professionals—to coordinate care plans and discharge strategies.
- Educate patients and their families on injury prevention, home exercises, and community resources for ongoing support.
- Maintain accurate documentation of treatments, progress notes, and discharge planning within electronic medical record (EMR) systems.
Skills
- Strong knowledge of physiology, anatomy, and medical terminology essential for accurate assessment and treatment planning.
- Proven experience in acute care settings such as hospitals or inpatient units, with familiarity in managing complex cases.
- Skilled in outpatient therapy environments including clinics and home & community care settings.
- Expertise in physiotherapy techniques such as trigger point therapy, orthopedics, geriatrics, pediatrics, and geriatrics.
- Ability to develop effective care plans that incorporate discharge planning for seamless patient transitions from hospital to home or community-based services.
- Experience working with EMR systems for documentation and patient management.
- Excellent patient care skills with a compassionate approach to diverse populations including elderly adults and pediatric patients.
- Knowledge of home health protocols and outpatient rehabilitation practices to deliver comprehensive therapy services across various environments. Pay: $55.00 - $70.00 per hour
