Summary

We are seeking a skilled Physical Therapist to join our team at Renew Home Health and Hospice. In this role, you will work closely with patients to develop personalized treatment plans aimed at restoring strength and mobility, ultimately helping them regain independence in their daily lives.

Responsibilities

• Evaluate patients' physical abilities and limitations through assessments.
• Develop individualized treatment plans to address patient needs and goals.
• Implement therapeutic exercises and interventions to improve strength, flexibility, and mobility.
• Educate patients and their families on treatment plans, exercises, and home care techniques.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Monitor patient progress and adjust treatment plans as necessary.
• Maintain accurate documentation of patient evaluations, treatments, and progress.

Requirements

• Valid state license to practice as a Physical Therapist.
• Strong knowledge of anatomy, physiology, and medical terminology.
• Experience in home health or hospice settings preferred but not required.
• Excellent communication skills for effective interaction with patients and families.
• Ability to work independently as well as part of a multidisciplinary team.
• Compassionate approach towards patient care with a focus on improving quality of life.
