Remedy Home Health Care is seeking a licensed Physical Therapist (PT) to join our Home Health team in Los Angeles. This Per Diem / PRN Physical Therapist role provides the flexibility of mobile therapy and home-based care, allowing you to work independently while making a meaningful impact on patients in your local territory.

Your Role: Home Health Physical Therapist

As a Home Health Physical Therapist (PT), you will deliver skilled physical therapy services in patients' homes, assisted living facilities, and senior communities throughout Los Angeles and surrounding areas.

Your work will focus on rehabilitation, fall prevention, mobility improvement, and restoring independence for patients recovering from injury, illness, surgery, or chronic conditions including dementia and geriatric health challenges.

Key Responsibilities

• Conduct in-home physical therapy evaluations and follow-up visits
• Develop individualized rehabilitation treatment plans
• Improve strength, balance, mobility, and functional independence
• Provide therapy to geriatric and senior patients in home-based care settings
• Educate patients and caregivers on safe movement, fall prevention, and assistive equipment
• Document visits in WellSky Kinnser EMR
• Collaborate with physicians, nurses, and interdisciplinary care teams
• Provide therapy in homes, assisted living facilities, and senior communities

Qualifications

• Licensed Physical Therapist (PT) – California
• CPR Certification (AHA approved)
• Minimum 1 year of clinical experience (Home Health experience preferred)
• Experience working with geriatric patients, dementia care, or senior populations preferred
• Strong communication, clinical judgment, and patient education skills
• Valid driver's license, reliable transportation, and auto insurance
