Schedule: Tuesday through Saturday
Pay: Up to $53/hour (Experience required)

Victoria Post Acute Care is a 120-bed skilled nursing facility located in El Cajon, California, dedicated to providing high-quality care through compassion, teamwork, and clinical excellence. Our team supports residents across long-term care, short-term rehabilitation, and memory care services throughout the East County community.

We are currently seeking an experienced Physical Therapist with skilled nursing or healthcare experience who is clinically strong, patient focused, and passionate about improving mobility and functional outcomes.

Position Summary

As a Physical Therapist, you will play a key role in helping patients improve mobility, strength, balance, and overall physical function. You will evaluate patients, develop individualized treatment plans, and provide hands-on therapy in a collaborative care environment.

This role is ideal for someone who enjoys working closely with patients, collaborating with interdisciplinary teams, and maintaining a high standard of care in a fast-paced environment.

Key Responsibilities

Evaluate patients and develop individualized treatment plans based on physical and functional needs
Provide therapeutic interventions to improve strength, mobility, balance, and coordination
Monitor patient progress and adjust treatment plans as needed
Collaborate with physicians, nursing staff, and interdisciplinary team members
Maintain accurate and timely documentation of treatments and patient progress
Educate patients and families on therapy goals and home exercise programs
Ensure compliance with all regulatory and facility standards

Qualifications

Active Physical Therapist license in California required
Minimum of 1 year of experience required
Skilled nursing facility (SNF) experience preferred
Strong clinical assessment and documentation skills
Ability to work independently and as part of a team
Excellent communication and patient care skills
Compassionate, dependable, and patient focused
