PYSICAL THERAPIST BENEFITS

PHYSICAL THERAPIST ESSENTIAL FUNCTIONS

· Provides clinical assessments for new patients and subsequent reassessments as needed.

· Creates Treatment Plans of Care.

· Provides therapy treatments.

· May provide supervision of physical therapy assistants.

· Prepares notes/reports for Case Conference.

· Works within a team to provide best practice care.

· Communicates adjustments of treatment plans with physicians.

· Logs complaints and incident reports and monitors for improvement.

PHYSICAL THERAPIST QUALIFICATIONS

· Cheerful customer service oriented.

· Current Utah Physical Therapist license (required).

· Experience in the home health and/or related settings.

· Demonstrated ability to apply critical thinking skills.

· Knowledge of and ability to navigate electronic medical records. HomeCare HomeBase experience a plus.

· Reliable transportation is required.
