At Hamilton Health Associates, we are one of only integrated wellness facilities in the area. We provide our patients with comprehensive care including Chiropractic, Physical Therapy, a Nurse Practitioner, Massage Therapy, and Psychology.
We're looking for a dynamic Physical Therapist to join our team in improving the quality of your patients by reducing pain, restoring mobility, and improving quality of life for patients with a team of professionals.
You will assess patient needs, develop personalized care plans, and implement evidence-based physoiotherapy techniques to promote healing and functional independence.

Responsibilities
- Conduct comprehensive assessments of patients’ physical conditions, including mobility, strength, and range of motion, utilizing your physiology and anatomy knowledge.
- Develop and implement tailored care plans that incorporate physiotherapy techniques such as trigger point therapy, orthopedics strategies, and geriatrics-focused interventions.
- Manage discharge planning processes to ensure smooth transitions from inpatient to outpatient or home & community care settings.
- Provide patient-centered care across various environments including acute care units, outpatient clinics, hospital wards, home health visits, and pediatric or geriatric populations.
- Educate patients and their families on injury prevention, rehabilitation exercises, and self-care strategies to promote long-term wellness.
- Document all patient interactions accurately within Electronic Medical Record (EMR) systems while maintaining compliance with medical terminology standards.
- Collaborate with multidisciplinary teams to optimize patient outcomes through coordinated care plans and ongoing progress evaluations.
Requirements
- Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
- Proven experience in inpatient hospital settings, outpatient clinics, or home health environments with a focus on patient care.
- Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques including trigger point therapy and orthopedics.
- Experience in discharge planning, care plan development, and working with diverse populations such as pediatrics or geriatrics.
- Familiarity with EMR systems for documentation and patient management.
- Ability to work effectively in fast-paced settings like acute care units or outpatient clinics while maintaining attention to detail.

Pay: $75,000.00 - $100,000.00 per year
