What You’ll Be Doing

• Evaluate and treat residents to improve mobility, strength, balance, and functional independence

• Develop and implement individualized, goal-driven treatment plans

• Document evaluations, treatments, and progress using Net Health and PointClickCare

• Collaborate with an interdisciplinary team to support resident-centered care plans

• Provide education to residents, families, and staff as appropriate

What We’re Looking For

• Active Pennsylvania Physical Therapist (PT) license, or eligibility to obtain

• SNF/LTC experience preferred, but new graduates are welcome and encouraged to apply

• Familiarity with Net Health and PointClickCare is a plus

• Strong clinical judgment, communication, and teamwork skills

• Passion for patient-centered rehabilitation and helping residents achieve their highest level of independence
