Physical Therapist

Duties

  • Assess patients’ physical conditions by evaluating mobility, strength, and functional abilities using comprehensive physiology and anatomy knowledge.
  • Develop and implement individualized care plans that align with patients’ goals and medical requirements, incorporating therapeutic exercises and modalities such as trigger point therapy.
  • Administer physiotherapy treatments across various settings including inpatient hospital units, outpatient clinics, home health visits, and community care programs.
  • Monitor patient progress regularly, adjusting treatment strategies to optimize outcomes and ensure safety.
  • Collaborate with multidisciplinary teams to coordinate discharge planning, ensuring seamless transitions from hospital to home or community care.
  • Document all patient interactions accurately within Electronic Medical Records (EMR) systems while maintaining compliance with healthcare regulations.
  • Educate patients and their families on injury prevention, physiotherapy exercises, and overall wellness strategies to support long-term health.

Experience

  • Proven experience in acute care hospitals, inpatient units, outpatient clinics, or home health environments demonstrating strong patient care skills.
  • Knowledge of medical terminology, physiology, and human anatomy essential for accurate assessments and treatment planning.
  • Hands-on experience with care plans tailored for diverse populations including pediatrics, geriatrics, orthopedics, and neurological conditions.
  • Familiarity with EMR systems for documentation and patient management.
  • Previous work in home & community care settings preferred to support patient independence outside clinical environments.
  • Skills in discharge planning and collaboration with healthcare teams to facilitate smooth patient transitions.

Pay: $46.00 per hour
