Physical Therapist

Duties

• Conduct initial and ongoing physical therapy evaluations in the patient’s home
• Develop and implement individualized treatment plans based on patient needs and physician orders
• Improve patient mobility, strength, balance, coordination, and pain management
• Provide gait training, transfer training, and fall prevention education
• Educate patients and caregivers on exercises, safety techniques, and use of assistive devices
• Monitor patient progress and adjust treatment plans as needed
• Complete all required documentation accurately and in a timely manner
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate care
• Ensure compliance with Medicare guidelines and agency policies
• Participate in discharge planning and patient outcomes evaluation

Qualifications:

• Active Physical Therapist (PT) license in the state of practice
• Graduate of an accredited physical therapy program
• Minimum of 1 year of clinical experience (home health experience preferred)
• Strong clinical assessment and decision-making skills
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively
• Valid driver’s license and reliable transportation

Pay: $100.00 - $110.00 per hour
