Responsibilities

  • Conduct comprehensive patient assessments to determine functional limitations and develop tailored care plans.
  • Deliver evidence-based physiotherapy treatments utilizing techniques such as trigger point therapy, orthopedics, and rehabilitation exercises.
  • Manage discharge planning processes to ensure seamless transition from hospital or inpatient care to home or community settings.
  • Collaborate with multidisciplinary teams to coordinate care for patients in acute care, outpatient clinics, or home health environments.
  • Educate patients and caregivers on injury prevention, physiotherapy exercises, and health maintenance strategies.
  • Maintain accurate documentation of patient progress using EMR (Electronic Medical Records) systems and ensure compliance with medical terminology standards.
  • Stay current with advancements in physiotherapy practices, including pediatrics, geriatrics, and specialized areas like orthopedics and sports medicine.

Requirements

  • Valid New Jersey licensure as a licensed Physical Therapist.
  • Proven experience in inpatient hospital settings, outpatient clinics, or home health care environments.
  • Strong knowledge of anatomy, physiology, medical terminology, and care plan development.
  • Hands-on experience with discharge planning, acute care protocols, and community-based care delivery.
  • Familiarity with EMR systems for documentation and patient record management.
  • Ability to work effectively across diverse patient populations including pediatrics and geriatrics.
