Remedy Home Health Care is seeking a licensed Physical Therapist (PT) to join our Home Health team in Los Angeles . This Per Diem / PRN Physical Therapist role provides the flexibility of mobile therapy and home-based care , allowing you to work independently while making a meaningful impact on patients in your local territory.

Our patients include seniors, geriatric patients, and individuals recovering from illness or injury , many of whom reside in private homes, assisted living communities, and senior living environments . Your Role: Home Health Physical Therapist

As a Home Health Physical Therapist (PT) , you will deliver skilled physical therapy services in patients’ homes , assisted living facilities, and senior communities throughout Los Angeles and surrounding areas .

Your work will focus on rehabilitation, fall prevention, mobility improvement, and restoring independence for patients recovering from injury, illness, surgery, or chronic conditions including dementia and geriatric health challenges .

Key Responsibilities

• Conduct in-home physical therapy evaluations and follow-up visits
• Develop individualized rehabilitation treatment plans
• Improve strength, balance, mobility, and functional independence
• Provide therapy to geriatric and senior patients in home-based care settings
• Educate patients and caregivers on safe movement, fall prevention, and assistive equipment
• Document visits in WellSky Kinnser EMR
• Collaborate with physicians, nurses, and interdisciplinary care teams
• Provide therapy in homes, assisted living facilities, and senior communities
  
  

Qualifications

• Licensed Physical Therapist (PT) – California
• CPR Certification (AHA approved)
• Minimum 1 year of clinical experience (Home Health experience preferred)
• Experience working with geriatric patients, dementia care, or senior populations preferred
• Strong communication, clinical judgment, and patient education skills
• Valid driver’s license, reliable transportation, and auto insurance
