Physical Therapist

Job Summary
We are seeking a dedicated and compassionate Physical Therapist to join Freedom Home Health. We are a 4-star Medicare certified agency who treats patients all over the valley of the sun, and we are seeking to cover post-op care (TKA/THA) and other medical conditions. The ideal candidate will provide high-quality patient care in patients' homes, Assisted Living Facilities, group homes and even hotel rooms for the transient patients. You will assess patients' physical conditions, develop personalized treatment plans, and guide them through rehabilitation exercises to improve mobility and quality of life. Your expertise in medical terminology, physiology, and specialized areas such as geriatrics, neurology and orthopedics will be essential in delivering effective therapy.

Responsibilities

• Conduct thorough evaluations of patients’ physical abilities and limitations.
• Develop individualized treatment plans based on patient assessments.
• Implement therapeutic interventions to enhance mobility, strength, and overall function.
• Educate patients and their families on treatment protocols and home exercise programs.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate documentation of patient progress and treatment outcomes.
• Stay updated on advancements in physical therapy practices and techniques.

Skills

• Proficient understanding of medical terminology related to physical therapy.
• Knowledge of physiology and its application in rehabilitation.
• Experience with home health therapy.
• Familiarity with geriatric and orthopedic patient needs.
• Strong communication skills for patient education and interdisciplinary collaboration.
