Overview:
We are seeking an experienced and compassionate Physical Therapist to join a collaborative interdisciplinary team dedicated to delivering exceptional, person-centered care for older adults. In this role, you’ll focus on helping participants maintain independence and improve their quality of life through individualized therapy and wellness programs.

Key Responsibilities:
- Perform physical therapy assessments in the clinic and occasionally in participants’ homes to identify care needs and personal goals
- Partner with the interdisciplinary team to design and update tailored care plans
- Deliver and document therapy interventions including exercise, mobility, and functional training for activities of daily living
- Apply therapeutic techniques such as massage, heat, hydrotherapy, or electrotherapy when appropriate
- Provide guidance to therapy assistants, caregivers, and families to ensure effective continuation of therapy and safe use of equipment
Qualifications:
- Bachelor’s degree in Physical Therapy required; master’s or doctoral degree preferred
- Two (2) years of experience in a clinical setting working with older or medically complex populations preferred
- Current California Physical Therapy license or license eligibility
- CPR and First Aid certification required (or must be obtained within 180 days of hire)

Pay: $115,000.00 - $140,000.00 per year
