Responsibilities

• Conduct comprehensive patient assessments to determine functional limitations and develop tailored care plans based on physiology and anatomy knowledge.
• Manage discharge planning processes by coordinating with multidisciplinary teams to ensure seamless patient transitions from hospital to home or community settings.
• Provide specialized care for geriatric, orthopedic, and neurological patients in home health environments.
• Document patient progress accurately using Electronic Medical Records (EMR) systems and communicate treatment plans clearly with patients and team members.
• Educate patients and caregivers on injury prevention, home exercise programs, and community resources to support ongoing recovery.

Requirements

• Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
• Experience in home care settings preferred.
• Strong knowledge of physiology, anatomy, orthopedics, geriatrics, and trigger point therapy techniques.
• Familiarity with EMR systems for documentation and patient management.
• Excellent patient care skills with the ability to develop effective care plans tailored to individual needs.
• Experience in discharge planning and working within multidisciplinary teams to ensure comprehensive patient support.
