Position Summary
We are seeking a licensed Physical Therapist to provide high-quality therapy services to patients recovering from surgery, injury, illness, or functional limitations. You will assess needs, develop individualized treatment plans, and deliver hands-on interventions to help patients reduce pain, restore mobility, and regain independence.
The ideal candidate is patient-centered, organized, clinically strong, and committed to delivering exceptional home-based care.
Key Responsibilities
Conduct comprehensive physical evaluations and functional assessments
Interpret patient histories, evaluations, and diagnostic findings to develop individualized treatment plans
Provide therapeutic interventions, including strengthening exercises, gait training, balance training, and manual therapy
Educate patients and families on home exercise programs, safe mobility, ergonomics, and injury prevention
Adjust treatment plans based on patient progress and evolving needs
Collaborate closely with nurses, physicians, and other healthcare professionals to coordinate care
Maintain accurate, timely, and compliant documentation in the EMR
Ensure adherence to federal, state, and agency regulatory standards
Stay current with evidence-based practices and new therapeutic approaches
Qualifications
Master’s or Doctorate degree in Physical Therapy from an accredited program
Successful completion of the National Physical Therapy Examination (NPTE)
Active Florida Physical Therapist license
Strong communication, interpersonal, and patient engagement skills
Ability to work independently and manage a home health caseload
Compassionate, detail-oriented, and committed to high-quality care
Proficient with computers and EMR documentation experience with ALORA Plus EMR preferred.
(Preferred) APTA-accredited residency training or prior home health experience
