Position Summary

We are seeking a licensed Physical Therapist to provide high-quality therapy services to patients recovering from surgery, injury, illness, or functional limitations. You will assess needs, develop individualized treatment plans, and deliver hands-on interventions to help patients reduce pain, restore mobility, and regain independence.

The ideal candidate is patient-centered, organized, clinically strong, and committed to delivering exceptional home-based care.

Key Responsibilities

  • Conduct comprehensive physical evaluations and functional assessments

  • Interpret patient histories, evaluations, and diagnostic findings to develop individualized treatment plans

  • Provide therapeutic interventions, including strengthening exercises, gait training, balance training, and manual therapy

  • Educate patients and families on home exercise programs, safe mobility, ergonomics, and injury prevention

  • Adjust treatment plans based on patient progress and evolving needs

  • Collaborate closely with nurses, physicians, and other healthcare professionals to coordinate care

  • Maintain accurate, timely, and compliant documentation in the EMR

  • Ensure adherence to federal, state, and agency regulatory standards

  • Stay current with evidence-based practices and new therapeutic approaches

Qualifications

  • Master’s or Doctorate degree in Physical Therapy from an accredited program

  • Successful completion of the National Physical Therapy Examination (NPTE)

  • Active Florida Physical Therapist license

  • Strong communication, interpersonal, and patient engagement skills

  • Ability to work independently and manage a home health caseload

  • Compassionate, detail-oriented, and committed to high-quality care

  • Proficient with computers and EMR documentation experience with ALORA Plus EMR preferred.

  • (Preferred) APTA-accredited residency training or prior home health experience
