Physical Therapist

Job Summary
We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. Your expertise will support a diverse patient population across inpatient, outpatient, home health, and community care settings. Responsibilities

• Conduct comprehensive patient evaluations to determine physical capabilities and develop tailored care plans.
• Implement therapeutic interventions including manual therapy, trigger point therapy, and exercise programs to promote recovery.
• Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless patient transitions across care settings.
• Monitor patient progress regularly, adjusting treatment plans as needed to optimize outcomes.
• Document all patient interactions accurately using electronic medical record (EMR) systems, maintaining compliance with healthcare regulations. ( Axxess)
• Educate patients and their families on physiotherapy techniques, injury prevention, and home exercise programs.
• Participate in home & community care visits to deliver consistent patient care across various environments.

Skills

• Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice.
• Experience in acute care hospitals, inpatient units, outpatient clinics, or home health settings.
• Proficiency with EMR systems for documentation and patient record management.
• Expertise in care planning for diverse populations including pediatrics and geriatrics.
• Familiarity with discharge planning processes and Home Health services.
• Skilled in orthopedic therapy techniques such as trigger point therapy and treatment of musculoskeletal conditions.
