Job Overview
We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. Duties
- Assess patients’ physical conditions by reviewing medical histories and conducting comprehensive examinations focused on physiology and anatomy knowledge.
- Develop and implement tailored care plans that incorporate physiotherapy techniques such as trigger point therapy, orthopedics, geriatrics, pediatrics, and outpatient rehabilitation.
- Guide patients through therapeutic exercises and manual therapy to restore function and alleviate pain.
- Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless patient transitions from hospital to home or community settings.
- Monitor patient progress regularly, adjusting treatment strategies as needed to optimize outcomes.
- Maintain accurate documentation of patient care using electronic medical record (EMR) systems, including detailed notes on treatment plans, progress reports, and discharge summaries.
- Educate patients and their families about injury prevention, physiotherapy exercises, and self-care techniques for home & community care environments.
Requirements
- Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
- Proven experience working in inpatient hospital settings, outpatient clinics, or home health environments.
- Strong knowledge of physiology, anatomy, medical terminology, and various physiotherapy modalities including trigger point therapy.
- Experience with discharge planning, care plan development, and patient care coordination across diverse populations such as geriatrics and pediatrics.
- Familiarity with EMR systems for documentation and patient management.
- Excellent communication skills to effectively educate patients about treatment protocols and recovery strategies.
- Ability to work independently as well as part of a multidisciplinary healthcare team in fast-paced environments like acute care hospitals or outpatient clinics.
