Duties

• Assess patients’ physical conditions by reviewing medical histories and conducting comprehensive examinations focused on physiology and anatomy knowledge.
• Develop and implement tailored care plans that incorporate physiotherapy techniques such as trigger point therapy, orthopedics, geriatrics, pediatrics, and outpatient rehabilitation.
• Guide patients through therapeutic exercises and manual therapy to restore function and alleviate pain.
• Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless patient transitions from hospital to home or community settings.
• Monitor patient progress regularly, adjusting treatment strategies as needed to optimize outcomes.
• Maintain accurate documentation of patient care using electronic medical record (EMR) systems, including detailed notes on treatment plans, progress reports, and discharge summaries.
• Educate patients and their families about injury prevention, physiotherapy exercises, and self-care techniques for home & community care environments.

Requirements

• Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
• Proven experience working in inpatient hospital settings, outpatient clinics, or home health environments.
• Strong knowledge of physiology, anatomy, medical terminology, and various physiotherapy modalities including trigger point therapy.
• Experience with discharge planning, care plan development, and patient care coordination across diverse populations such as geriatrics and pediatrics.
• Familiarity with EMR systems for documentation and patient management.
• Excellent communication skills to effectively educate patients about treatment protocols and recovery strategies.
• Ability to work independently as well as part of a multidisciplinary healthcare team in fast-paced environments like acute care hospitals or outpatient clinics.
