Your Role as a Physical Therapist:

You will play a key role in enhancing residents’ mobility, function, and quality of life. You’ll evaluate, develop, and implement treatment plans tailored to each individual’s goals—helping them regain independence and strength.

Key Responsibilities:

  • Perform initial and ongoing assessments of residents' physical conditions and functional abilities

  • Develop and carry out individualized treatment plans to reduce pain, restore mobility, and prevent further injury

  • Document all patient interactions, assessments, and progress in the Net Health EMR system (preferred)

  • Collaborate with caregivers, nursing staff, and interdisciplinary team members

  • Provide patient and caregiver education on treatment strategies and home programs

  • Monitor and adjust treatment based on patient response and progress

  • Ensure compliance with all federal, state, and facility policies and procedures

Qualifications:

  • Current PA license as a Physical Therapist in good standing (required)

  • Experience in SNF/long-term care settings preferred

  • Knowledge of current therapy best practices and protocols

  • Experience with Net Health EMR preferred

  • Ability to work with patients of varying conditions and personalities

  • Strong communication, organizational, and interpersonal skills
