Duties

  • Conduct comprehensive patient assessments to evaluate physical function, mobility, and pain levels using medical terminology and anatomy knowledge.
  • Develop and implement tailored care plans that incorporate physiotherapy techniques such as trigger point therapy, orthopedics strategies, and geriatrics approaches.
  • Deliver hands-on therapy sessions in inpatient, outpatient, hospital, or home & community care settings to promote healing and restore function.
  • Manage discharge planning processes by coordinating with multidisciplinary teams to ensure seamless transitions from hospital to home or community environments.
  • Monitor patient progress regularly, adjusting treatment strategies based on physiology responses and clinical observations.
  • Document all patient interactions accurately within electronic medical record (EMR) systems while maintaining compliance with healthcare regulations.
  • Educate patients and their families on injury prevention, physiotherapy exercises, and self-care techniques to support long-term wellness.

Qualifications

  • Valid licensure or certification as a licensed Physical Therapist with relevant state or regional authorities.
  • Proven experience in acute care, inpatient hospital settings, outpatient clinics, or home health environments.
  • Strong knowledge of physiology, anatomy, medical terminology, and specialized therapies such as trigger point therapy and orthopedics.
  • Demonstrated ability to develop effective care plans for diverse populations including pediatrics and geriatrics.
  • Experience with EMR systems for documentation and patient management.
  • Excellent communication skills to collaborate with healthcare teams and educate patients effectively.
  • Prior experience in discharge planning and managing care transitions across various healthcare settings is highly desirable. Pay: $40.00 - $50.00 per hour
