Summary

We are seeking a Physical Therapist to join our team at Kokua Healthcare. In this role, you will provide high-quality physical therapy services to clients in their homes, helping them achieve their rehabilitation goals and improve their overall well-being.

Responsibilities

• Conduct thorough assessments of patients' physical abilities and limitations.
• Develop personalized treatment plans based on individual needs and goals.
• Implement therapeutic interventions to enhance mobility and function.
• Educate patients and families on treatment plans and home exercises.
• Monitor patient progress and adjust treatment plans as necessary.
• Collaborate with other healthcare professionals to ensure comprehensive care.
• Maintain accurate documentation of patient care and progress.

Requirements

• Valid state license to practice as a Physical Therapist.
• Strong knowledge of medical terminology and physical therapy techniques.
• Experience in home health or community care settings preferred.
• Ability to work independently and manage time effectively.
• Excellent communication skills for interacting with patients and families.

Pay: $35.00 - $150.00 per hour
