Job Summary
We are seeking a motivated and energetic Physical Therapist to join our dynamic team. In this vital role, you will assess, develop, and implement individualized care plans that promote recovery, mobility, and independence for patients across various settings. Your expertise will help improve patient outcomes through targeted physiotherapy interventions, discharge planning, and collaborative care.

Duties

Reviews the physician’s referral and the client’s medical history and current diagnosis, and performs the initial evaluation to identify the clients’ needs and priorities. Develops the individualized treatment plan to maintain, restore, or improve physical functioning, alleviate pain, and prevent physical dysfunction.

Administers standard diagnostic and prognostic examination of muscle, nerve, joint and functional ability during the evaluation and assessment process, identifies needs, discusses with the patient the proposed plan of treatment and long and short range goals to be achieved and their role in their achievement and obtains informed consent.

Implements treatments based on the plan of treatment, conducts actual treatment programs using various modalities such as: administers active, passive and resistive therapeutic exercises; local or general massage; muscle retraining and re-education; gait training, muscle strengthening, therapeutic massage and other activities according to the plan of care; corrective exercises and neuromuscular coordination; design and construct, or direct construction of specialized testing and treatment equipment; explains treatment program and functional training using various devices and equipment; fit patient for orthopedic braces, prosthesis and supportive devices; teach transfer, lifting and positioning techniques; train caregivers in supportive techniques for transferring and positioning patients and in use of assistive and supportive equipment and devices.

Instructs client in walking, standing, balance, use of crutches, cane or walker; care of braces and artificial limbs.

Provides instruction to the client and responsible caregivers regarding the home exercise program. Instructs them regarding exercises to be continued at home, and interprets for them the significance of physical therapy services. Provides handouts of exercise program to facilitate clients remembering and complying with regimen.

Conducts on-going re-evaluation of clients’ responses to therapy and progress towards goals and discusses with the appropriate physician and other direct care providers to coordinate and collaborate on revisions in the plan of treatment.

Documents all activities performed in compliance with regulatory requirements and the agency’s documentation policies and procedures. Submits documentation within the established timeframe and guidelines re timeliness, appropriateness, completeness, relevancy and in compliance with the plan of care.

Provides general supervision of physical therapy treatment activities assigned to LPTA’s , performs, the initial evaluation, discusses the plan of treatment to be followed, conducts supervisory visit every 12th visit or once every 30 days to evaluate the client’s progress, conducts the re-certification evaluation, discharge or resumption of care evaluations, and maintains oversight of the client’s care during the full episode of treatments by the agency, communicating with the LPTA to discuss the client’s response and progress towards goal achievement..

Must meet Human Resources requirements; such as education, licensure, continuing education, criminal background checks, CPR, health clearance, in-services and competences on hire and to maintain employment.

Education

Bachelor of Science degree from a Physical Therapy curriculum approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of the American Medical Association and the American Physical Therapy Association

Experience

Two years of physical therapy acute care experience. One year of home health physical therapy experience.

Certificate/License

Current Physical Therapy license certification by the Common wealth of Virginia Valid Drivers License with certificate of automobile insurance.

Pay: $39.00 - $52.00 per hour
