Physical Therapist (PT)

Status: Full-Time

Lakewood is hiring a Full-Time Physical Therapist (PT) to join our therapy team. This role plays a major part in resident mobility, fall prevention, strength building, transfer safety, and overall functional independence.

This position works closely with nursing and interdisciplinary teams to help residents improve mobility, safely perform daily routines, recover following hospitalization or injury, and maintain the highest level of independence possible.

What You’ll Do

• Evaluate residents and develop individualized treatment plans focused on mobility, strength, balance, endurance, and functional outcomes
• Provide therapy services for residents recovering from surgery, hospitalization, falls, fractures, weakness, and functional decline
• Assist residents with gait training, transfer training, balance, wheelchair mobility, and strengthening exercises
• Help improve resident safety and reduce fall risk throughout the building
• Collaborate closely with nursing and interdisciplinary teams regarding mobility status, discharge planning, and resident progress
• Complete documentation and evaluations accurately and efficiently
• Support residents through both short-term rehabilitation and longer-term care needs

Qualifications

• Active Colorado Physical Therapist license required
• Previous long-term care, post-acute, or rehabilitation experience preferred
• Strong clinical judgment and communication skills
• Ability to work independently while contributing to a collaborative team environment
