POSITION DESCRIPTION

POSITION: Physical Therapist

REPORTS TO: Director of Patient Services

POSITION SUMMARY: Physical Therapy Services are provided to individuals who demonstrate the presence of a functional limitation. Physical therapist’s role is to provide skilled intervention to improve the patient’s level of functioning in self-care, mobility, safety, range of motion and to establish a safe and effective maintenance program. Examples of therapy services include assessment of needs and development of a therapy program for the patient.

QUALIFICATIONS:

  • Currently licensed and registered with the NYS Department of Education as a Physical Therapist.
  • Graduation from a Physical Therapy curriculum approved by the American Physical Therapy Association or the Council on Medical Education of The American Medical Association.
  • One your physical therapy experience in an acute health care setting, home health care experience desirable.
  • Good organizations, interpersonal and communication skills including demonstrated abilities to:
  • Read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.
  • Speak effectively before groups or customers or employees of the Agency.
  • Read, analyze and interpret general business correspondence.
  • Effectively present information and respond to questions from groups of managers, patients, customers, and the general public.
  • Computer literate, desired

DUTIES / RESPONSIBILITIES

Intervenes in the restorative health care plan upon receipt of a request for an evaluation and treatment of a physical disability. Submits written initial evaluation for inclusion in patient’s world.

Perform clinical tests to discover functional limitations and the potential for restoration. Develops a plan of care with restorative and maintenance goals to meet identified needs.

Applies physical therapy procedures to achieve identified goals, (i.e, therapeutics exercises, functional training).

Instructs patients, family and other health team personnel in the use of assistive modalities and appropriate physical therapy procedures.

Observes, records and reports to the authorized medical practitioner and nurse, the patient’s reaction to treatment and any changes in the patient’s condition.

Works with the health team to shorten the patient’s recovery from illness and incorporate the management of residual disability.
